APFED’s Physician Locator is provided by APFED for use by the general public as a reference source for information on self-identified physicians with practices offering care for patients with a variety of eosinophilic disorders. These practices vary widely in size, training, and services offered. The information was obtained from a voluntary survey of physicians that are professional members of APFED. All data in the APFED Physician Locator is self-reported and has not been independently verified by APFED. All users should verify any information received from the Physician Locator with the listed physician or center. Decisions regarding medical care should be made with your healthcare provider.

The Physician Locator provides self-reported information on:

• Subspecialty and board certification;

• Practitioner’s area of interest;

• Types of patients covered in the practice;

• Total number of patients with eosinophilic disorders seen per month;

• Total number of half-day clinic sessions per week devoted to patients with eosinophilic diseases;

• Number of years spent caring for patients with eosinophilic diseases;

• Services offered specific to eosinophilic diseases;

• Wait times for new patient appointments;

• Availability of multi-specialty collaborative centers;

• Location; and

• Contact information for practitioners.

The inclusion of a physician or center in the Physician Locator does not imply APFED’s recommendation or endorsement, nor does the omission of a physician or center in the Physician Locator imply APFED’s disapproval. The Physician Locator is not intended as a tool for verifying the credentials, qualifications, or abilities of any physician or center contained therein, nor does it indicate that a physician or center meets standards for specialized eosinophil-associated disorders care. All recommendations made within the Physician Locator are made by independent third-party users of the service and not by APFED.

APFED shall not be liable to any person or entity for any claims, damages, liability or other loss including, without limitation, any liability for injury or other damage resulting from any use of, or reliance on, any information contained in the Physician Locator or any recommendation made by any independent third-party user. APFED makes no representations or warranties about the accuracy, reliability, completeness, currentness, or timeliness of the content, services, text, graphics, links, or communications provided on or through use of the Physician Locator.

You are prohibited from using, downloading, republishing, selling, duplicating, or “scraping” for commercial or any other purpose whatsoever, any of the data listings or other information in the Physician Locator, in whole or in part, in any medium whatsoever. By using this service you are agreeing to the terms and conditions set forth above.

Note: Participation in the Physician Locator is open to all physicians with practices involving eosinophilic diseases in the United States, Canada, Europe, Australia, and Asia.
